Rachel Ritter,
Arizona State University Soccer Team

Ted Bezemer, PT, MS, OCS, SCS

Ted is founder of ProForm Athletics, a team of multi-disciplinary health and performance experts in Park City that provide professional-level rehabilitation, athletic development and performance solutions to individuals and teams of all ages, goals and ability. 

Ted is a licensed Physical Therapist, Certified Strength and Conditioning Specialist, Titleist Performance Institute medical professional and golf swing biomechanist with over twenty years of experience in the sports medicine field.  He has worked closely with some of the best Orthopedists in the country and treated clients and patients ranging from recreational to Olympic and professional athletes.
 
Ted has a Masters of Science Degree in Physical Therapy and training from world-renowned Harvard University affiliates, Massachusetts General Hospital Institute of Health Professions and Brigham & Women’s Hospital in Boston where he specialized in orthopedics and sports medicine. 
Most notably, Ted is a Board Certified Clinical Specialist in both Orthopedics and Sports Injury.  This is in recognition by the American Physical Therapy Association to physical therapists with advanced clinical knowledge, experience and skills in a special area of practice.  Ted is the only physical therapist in the state to have achieved board certifications in both orthopedic and sports clinical specialties.

Jason Morgan, BS, CPT

Jason is ProForm Athletics Director of Team Strength and Conditioning.  He is a 2008 graduate of the University of Utah with a degree in Exercise and Sport Science and is certified with the National Exercise and Sports Trainers Association. 

Unique to most trainers, Jason has a very thorough understanding of the science and physiology of strength and conditioning.  Additionally, he has significant experience working in a physical therapy setting where he assists patients with their orthopedic and sports injury rehabilitation programs. 

Jason advocates the Canadian Sport for Life Long-term Athlete Development Model with his youth athletes and sports training programs.  He is a former collegiate baseball player and is very passionate about the sport and coaching.  He currently oversees the Park City High School team’s strength and conditioning program and travels with the team as an assistant coach and batting instructor.

Jason is an avid skier and mountain biker.  With a formal background in sports, a broad perspective in managing injury and avid coaching skills, Jason can help you reach the next level in any athletic pursuit.


Maureen Murtaugh, PhD, RD

Maureen provides nutritional consultation to all ProForm Athletics individuals and teams who want the best in customized dietary programming and education.  She is the ultimate resource for accurate, scientifically researched information to answer your nutritional questions and accommodate any need. 
In an area that’s significance in performance and health is often oversimplified and underestimated, Maureen can give you nutritional counseling, develop a customized program and help you sift through the abundance of nutritional misinformation that we are repeatedly bombarded with through media and from self-proclaimed experts.
Maureen’s education and experience far exceeds that of the typical nutritionist.  Dr. Murtaugh completed her PhD in nutrition at the University of Connecticut in 1986.  Since then, she has held multiple faculty positions teaching dietitians at the Master’s Degree level.  During her many years of clinical practice, Maureen has conducted diet-related research and authored or co-authored over 50 publications. 
Before joining an internationally recognized cancer epidemiology group at the University of Utah, Maureen spent three years as a post-doctoral fellow in epidemiology at the University of Minnesota.  She is also currently a member of the board of editors of the Journal of the American Dietetic Association.  This further enforces her use of only objective and clinically significant research with her practice and nutritional program development.

Maureen is able to translate state of the art research into practical, healthy food choices and cooking styles. She can tailor recommendations to address a number of issues including heart disease, kidney disease, diabetes, cancer, weight loss, women’s nutrition, and sports performance.   She is a tremendous asset to the ProForm Athletics team as she can provide you with a nutritional approach to achieve your goals, regardless of age, gender or medical condition. 

Courtney Meeboer, BA

Courtney is a mat and machine certified Pilates instructor with over seven years of teaching experience.  She holds certifications with both Quantum Fitness and Core Dynamics Pilates which gives her a very broad scope of training and practice. 

Courtney has a Bachelors Degree in Visual Performing Arts and Dance from the University of Maryland.  She has experience in many different styles of dance, including modern, ballet, jazz, and tap.  In addition, Courtney competed for nine years in gymnastics and was a track and field college scholarship athlete.  She is very effective at integrating this diverse background into her training programs and instruction.

We highly recommend including mat and machine Pilates into your conditioning regimen to improve body symmetry and performance.  Courtney is available for private, partner and group Pilates instruction at the location of your of choice.  You will find she provides excellent instruction combined with a very supportive and personable flare.


Jen Sayers, RYT

Jen is available for private and group yoga instruction at the Club or in your home, accommodating persons of all goals and levels of experience.  Jen is a certified Kripalu yoga instructor and registered with the highly accredited yoga alliance at the 200 hour level.

Kripalu Yoga thrives as a practical, accessible, and contemporary approach to yoga, embraced by many in its design to adapt to all body types, ages, fitness levels, and interests.  Jen’s pursuit of Kripalu stems from how the practice takes into account people’s individuality and personal expression.  And like with other styles of yoga, students notice the effects of practice from the first time they step onto their mats.

Jen approaches yoga instruction as an opportunity to assist the student in building a better, stronger, and more in depth relationship with themselves.  With warmth and clarity she balances the building of physical strength and stability while learning how to relax overworked areas to increase flexibility.  Students leave Jen’s classes feeling rejuvenated and relaxed. 

Jen sees herself as a yoga facilitator, reminding students that their practice is their own and she is merely a guide.  Through postures, breath work and deepening the mind-body connection students will learn to listen to their own bodies to make safe and healthy choices both on and off the mat.  

Creed Stobbe, MS

Creed is our Sport Psychologist and mental performance “Guru.”  He works with individuals and groups of all goals and abilities, providing the tools to succeed with all your endeavors.  Creed’s unique style of psychological performance enhancement has been honed and refined through years of training and application to specifically meet your needs.

Creed has a Masters of Science Degree in Sport Psychology from the University of Utah, under the guided direction of some of the world’s foremost Sport Psychologists.  Areas of specialized Sport Psychology training include psychometric assessment, arousal regulation, visualization training and routine formation.  Creed can also help you with your goal setting, adherence, relaxation, concentration and self-confidence.

His diverse background and use of the “Guillotine” method of Sport and Performance Psychology enables him to contribute to the health and performance of individuals in a variety of disciplines.  He has worked with competitive cyclists, skiers, snowboarders, amateur and professional golfers, as well as other groups and individuals trying to achieve their goals full potential.

Creed believes that in conjunction with a sound physical training and nutrition program, the mind can be fully engaged, and disengaged, to allow the body to achieve heights seldom otherwise experienced.
 

Anita Merbach, HFI, LMT

Anita is a Certified Health and Fitness Instructor through the American College of Sports Medicine.  She is available for fitness training, mountain biking and X-country skiing instruction at your convenience.  Anita’s strong conditioning, massage and athletic background provide her a very broad and unique combination of skills to offer her clients.

Anita has experience training and instructing all levels of athletes.  She has many athletic pursuits and is particularly interested in track and distance running program development.  She also has worked extensively in the rehabilitation setting, utilizing her exercise and massage backgrounds to restore and maximize patient’s function.

Anita is also an accomplished mountain biker, road cyclist and Nordic skier.  Her experience includes five years each of elite level women's cross country mountain bike and road bike racing.  In the summer she is a competitive runner and during the winter months she continues to participate in Nordic ski racing.  

A mountain biking coach and former PSIA Nordic ski instructor, Anita can not only help you condition optimally for these activities, but also provide the professional instruction and technique required to put it all together.

Sebastian Gobba,
Uruguayan National  Basketball Team

Sebastian began his basketball career playing for the under 19, under 21 and senior Uruguayan National Teams. 

He won the state title with Mount Vernon Academy in Salt Lake City and went on to play three years at Pacific College in Oregon where he averaged 8 pts, 6 rebounds and 1.2 blocks a game.

Rachel is a freshman soccer palyer at Arizona State University.

She is an exceptional student athlete.  Rachel's accomplishments include being a member of  the Utah ODP for 4 years, Utah All-State Team 4 years, and a recruited member of ASU's Women's Soccer team.  Rachel is also an exemplary student.  While at Park City High School she maintained a 3.78 GPA. Currently enrolled at Arizona State University she has a 3.52 cumulative GPA.

Janna Mann, PT, DPT

Janna received her doctorate in physical therapy from the University of Utah in 2008.  She has further training in integrated movement techniques and exercise.  Janna has rehabilitated an entire spectrum of individuals, from Olympic athletes to weekend warriors to 83 year old ski bums with a variety of injuries.

Through her experience, she has become aware of the need and benefit of adopting a preventative exercise approach to improve performance and decrease likelihood of injury.
Janna excels at applying her rehabilitation knowledge as a strength and conditioning coach. 
Her philosophy with training clients is that it’s best to integrate a thorough orthopedic exam into a personalized program that combines the client's goals, activities, and abilities.

A former Division I softball player, Janna enjoys playing on the ProForm Athletics softball team as well as skiing, mountain biking, and hiking.  Janna provides excellent strength and conditioning instruction, treatment for injuries and can help restore and improve athletic performance at every level of ability.



Merilee Johnson  LMT, NCMT

Merilee plays an integral part of the ProForm Athletics model in a collaborative effort with our therapists and trainers to solve movement impairment disorders and to maximize the health and performance for all our athletes and clients. 

A life long athlete in team sports, skiing and cycling, Merilee began a rowing career on the Potomac River while attending Georgetown University.  Pursuit of this sport at the elite and international levels of competition led her to follow an interest in the human body, mechanics, and kinesiology.

Merilee has over twenty years of massage therapy experience.  She has worked on many elite level athletes in professional cycling, alpine skiing, top professional Ironman competitors and teams, professional golfers, Race Across America teams, professional beach volleyball players and adventure racers.

Additionally, Merilee’s experience expands into detailed clinical work as well; having worked with physical therapists and experts in sports medicine and rehabilitation of chronic pain conditions and spinal injuries.  Combining this background with massage therapy gives her a unique awareness of the body’s systems and how they inter-relate with each other, inside and out.
